I understand that by working under the Umbrella of Loving
Investments LLC DBA Open Door MultiServices, I will be charged
10% of all my income.

I understand that after 2 terminations from any client it will be
extremely difficult for me to obtain a new client to service
(enforced by Arise and not Open Door MultiServices).

I understand that if I am terminated from one client, Open Door
MultiServices reserves the right to terminate me from their
umbrella allowing me 30 days to either incorporate or locating
another VSC under which I would like to work.

I am willing to work with Open Door MultiServices for a minimum
of 6 months. After that time I can choose to continue or I will
provide a 30 day notice to advise that I will be incorporating or
joining another VSC.

I understand that after having successfully serviced my clients
that I have acquired for 8 months maintaining a 95% or higher
Adherence, Quality of 90% of higher and fulfilling my weekly
commitments; my training fees for the client will be reimbursed.

I understand that I am contracting through Open Door
MultiServices to provide services to Arise so that I do not have to
incorporate at this time. I am still a contractor and not an
employee.

I understand that once I pay for and enroll in a client training if I
drop that client training there will be a 4 week waiting period
before I am able to enroll in any training class (this is enforced by
Arise not Open Door MultiServices).

By providing the above infomation, I authorize Loving
Investments LLC d/b/a Open Door MultiServices to add me under
their umbrella so that I can service clients with Arise.

We are all a work in progress and if at anytime I have any
questions, comments or concerns I will address them directly
with Open Door MultiServices allowing them the opportunity to
bring a resolution.

I understand that this opportunity does not provide paid training
and client training may take up to 5 weeks.

I understand that I will receive a 1099 to file my taxes.
